Forms Used in KOOP (IME) Training

You can access the current forms used within the YTÜ KOOP (IME) Model via the links below.

1) Application form and declaration of commitment:

- FR-1726-On-the-Job Vocational Training Application and Preference Form (referred to as Annex 1 in the On-the-Job Vocational Training Guidelines)

- FR-1727-On-the-Job Vocational Training Student Declaration and Commitment Form

2) Form to be completed upon becoming a KOOP student:
